Dear,

 

I am writing to inform you that I shall be absent from [DATE] to [DATE]due to a medical emergency where the doctor has advised me to take rest.

 

I apologize for this sudden decision. However, feel that it will not be beneficial for the company if I arrive at work not being in my best self to take care of my tasks. Please consider this letter as my formal request for a medical leave from the office. I will be back as soon as I recover.
